Account Number
Find your account number on your notice from the Backflow Compliance Office or in the upper right corner of your bill.
Service Address
This is the same as the Service Address listed on your water bill. It is based on the location of the water meter,
which may have a different address than the building(s) or unit(s) served by the water service line. After entering
your account number please confirm you are looking at information for the correct location.
Shut Off Due Date
The date on or after which water service may be shut off for non-compliance. Customers who fail to have required
tests performed or non-functional devices repaired are subject to shut-off. A Shut off Due date is created
when we deliver a Backflow Compliance Final Notice to the property. Shut off Due dates consider the last test date
and test status (pass/fail) and may not always correspond to the Test Due date shown below.
Important Dates
Test Due
Backflow prevention assemblies and low pressure sustaining devices on booster pumps must be tested at least
every 12 months. Due dates are rescheduled for the same date the following year once we receive a passing
test report. The test due date may be periodically adjusted based on the actual date of testing if significantly
early or late. If a device fails, repairs and a new test are required within 21 days.
Past Due Notice Issued
This is the date that the letter titled "IMPORTANT NOTICE, YOUR PROPERTY IS IN VIOLATION OF CITY CODE 1113.01"
was mailed to the site (and billing address if different from the site address).
Final Notice Issued
This is the date that the Backflow Compliance Final Notice was delivered to the site (and mailed to the billing
address if different from site address).
Device Information
Type
Standard device type. There are different types of devices required depending on the potential
contaminant(s). PVB, RP, RPDA, DC, DCDA, and AG are types of backflow prevention assemblies. SWITCH indicates
a low pressure sustaining device on a booster pump.
Size
The nominal pipe diameter of the device.
Last Test History
Date
Testers have 5 days from the test date to submit reports to us using an on-line system. We process test
reports within 2 business days after they are submitted on line by your tester. If after one week your test
results do not appear on this site please contact your tester first. You may also submit a copy of your test
report directly to our office at 918 Dublin Road, Columbus, OH 43215, or by e-mail at

or by fax at (614) 724-0790.
Testers receive a unique confirmation number from our on-line system when your test is submitted. Before
contacting our office to report problems with test submittals, please obtain the confirmation number from
your tester to verify the report was submitted.
Tested By
Company who performed last test. You may hire any tester who is currently approved by our office to test
backflow prevention assemblies. A current list, updated weekly, is available at
www.columbus.gov/backflow/consumers. Please note
that approval to test is granted to individuals not to companies. Tests not performed by an approved tester
will not be accepted by our office.
Test Result
Pass, Fail, or ND (not done/incomplete submittal). Devices that failed the test should be repaired
immediately. Repairs must be made within 21 days to avoid potential shut off of your water service. For failed
tests of a device that protects against a severe health hazard water service may be shut off immediately until
the issue is resolved.
Other
Protection From
Backflow protection for the water service lines from the public water main is most typically required when either
a specific site use (e.g. medical facility, car wash, restaurant et.al) or a specific hazard (lawn irrigation,
auxiliary water source (e.g. a well), fire suppression system, et. al.) exists. Local plumbing code may also
require isolation of a non-potable system (e.g. fire suppression system) from the private potable plumbing. This
field describes why the backflow preventer is required.
